What am I required to pay for?
- If you are unable to keep your scheduled appointment, please notify us 24 business hours before your appointment so we can offer the appointment to another patient. If you have any questions, please contact us.
Follow-up Appointments
- Any follow-up appointments, whether in-office or by phone, are scheduled based on the complexity of your health concerns. These appointments are billed in 15-minute increments. If your scheduled appointment exceeds the allotted time, Dr. Lizie will usually round the time down rather than up when possible. Please let us know if you have time constraints surrounding your scheduled appointment.
Medical Care
- Our practitioners are not medical doctors and cannot supervise medical emergencies. If you feel you need medical care, we urge you to visit your doctor or the nearest urgent care or emergency room. If you live in our local area, we can offer a list of licensed medical providers you may wish to consult.
Phone Appointments
- All phone appointments are treated the same way as an in-office appointment. Dr. Lizie will call you at the number you provide within 10 minutes of your scheduled time. If for some reason, she is unable to reach you or will be delayed longer than 10 minutes, one of our office staff will contact you to either reschedule or advise you of the updated time of her call.
- All phone appointments require payment with a credit card so please be sure to provide our staff with that information prior to your appointment.
Shipping
- If you are unable to come to pick up your supplements, they will be drop shipped to you. Orders received before 12pm Monday - Thursday will be shipped the same day. Orders after noon Monday - Thursday will ship the following business day. Orders are not shipped on holidays or during office closings.
Supplement Re-ordering
- When working with Dr. Lizie, we require that you purchase your supplements directly from our office or online dispensary so we can ensure you are receiving the correct herbal and nutritional products and are taking them at the recommended dosages. We closely monitor your reorder status to ensure you are following the correct protocol to obtain an optimal outcome.
Product Returns
Please refer to the items below for information about product returns.
- Pre-approval is required for all returns.
- Must be within 30-days of purchase.
- Refrigerated items cannot be returned.
- A 15% restocking fee, less shipping and handling, will be applied to unopened, un-refrigerated returns.
- Prepaid tests can be returned within one year of purchase if returned unused and unopened.
- Returns other than prepaid tests must be made within 30 days of purchase.
- Special orders cannot be returned.
What am I required to pay for?
- Consultation fees are discussed prior to your appointment and collected on the day of service.
- Supplement or product purchases are at your discretion.
