Policies & Procedures for Nutritional Consultations
What am I required to pay for?
- If you are unable to keep your scheduled appointment, please notify us 24 business hours before your appointment so we can offer the appointment to another patient. If you have any questions, please contact us at (978) 846-1964.
- Any follow-up appointments, whether in-office or by phone, are scheduled based on the complexity of your health concerns. These appointments are billed in 15 minute increments. If your scheduled appointment exceeds the allotted time, Dr. Lizie will usually round the time down rather than up when possible. Please let us know if you need to have your appointment adhere strictly to the allotted time (I.e. 15 minutes, 30 minutes, etc.) If you have any questions, please feel free to contact us at (978) 846-1964.
- Our practitioners are not medical doctors and cannot supervise medical emergencies. If you feel you need medical care, we urge you to visit your doctor or the nearest emergency room. If you live in our local area, we can offer a list of licensed medical providers you may wish to consult.
- All phone appointments are treated the same way as an in-office appointment. Dr. Lizie will call you at the number you provide within 10 minutes of your scheduled time. If for some reason, she is unable to reach you or will be delayed longer than 10 minutes, one of our office staff will contact you to either reschedule or advise you of the updated time of her call.
- All phone appointments require payment with a credit card so please be sure to provide our staff with that information prior to your appointment.
- If you live outside our local area or are unable to come in to pick up your supplements, we will be happy to drop ship them to you. Orders received before 12pm Monday through Thursday will be shipped the same day. All other orders will go out the following business day. Orders are not shipped on Friday – Sunday, holidays or during office closings. The drop ship fee of $7 per order is waived on all orders over $200.
- When working with Dr. Lizie, we require that you purchase your supplements directly from our office or using your online portal so we can ensure you are receiving the correct herbal and nutritional products and are taking them at the recommended dosages. We closely monitor your reorder status to ensure you are taking the right number of supplements in order to obtain an optimal outcome.

- Pre-approval is required for all returns
- Must be within 30-days of purchase
- Refrigerated items cannot be returned
- A 15% restocking fee, less shipping and handling, will be applied to unopened, un-refrigerated returns
- Prepaid tests can be returned within one year of purchase
- Returns other than prepaid test returns must be made within 30 days of purchase
- Special orders cannot be returned
What am I required to pay for?
- For each appointment in our office, you are only required to pay for the consultation costs*. Any supplement or herbal purchases are at your discretion.
- *The fee for your initial consultation is $250 for one hour. The length of follow-up appointments is based on the number of lab tests to review and the complexity of your health care issues.
